| The SUHFARS Membership application is a
PDF Form that you can either download or fill out online.
After the form is completed, you will need to print it and
then mail the completed form to W8DCD along with your dues
payment. You must print the filled out form, it cannot be saved online. Once the form is closed, all of the filled in information is lost. Download or fill in the form here. Print the form, then please mail
your application, along with your check payable to SUHFARS to: |
